Making a referral to Midlin is fairly easy.

  2. Complete and return to Midlin by email (send to manager@mid-lin.co.uk)

  3. Print and complete by hand and return to MidLin Day Centre, 59 Pitkerro Drive, Dundee, DD4 8AT

  4. We will then contact to to progress your application.
